Rezform ("we", "us", "our") is committed to protecting your privacy and the privacy of the data collected through our service. This Privacy Policy explains how we collect, use, disclose, and safeguard information when you use our service.
This policy applies to two types of users:
- 1. Restaurants: Users who sign up to Rezform to create capture pages and collect phone numbers from their customers.
- 2. Customers/Subscribers: Individuals who enter their phone numbers on capture pages created by Restaurants.
Rezform is operated by MV TEST SOLUTIONS LIMITED. We are registered with the UK Information Commissioner's Office (ICO) under reference number ZC041502.
Data Controller Information
For Restaurants (Rezform account holders):
MV TEST SOLUTIONS LIMITED is the data controller for your account information (email, password, profile data).
For Customers/Subscribers (people signing up via capture pages):
The Restaurant who owns the capture page is the data controller for your phone number. They determine how your data will be used and are responsible for contacting you. Rezform acts as a data processor on behalf of the Restaurant, providing the technical infrastructure to collect, store, and verify phone numbers.

Information for Customers/Subscribers
If you entered your phone number on a capture page, please read this section carefully:
What data is collected?
Your phone number (including country code) and the date/time you signed up. No other personal information is collected through capture pages.
Who has access to your phone number?
The Restaurant who owns the capture page has access to your verified phone number. They can view, export, and use this data to contact you via SMS. Rezform staff may access data for technical support or to investigate abuse, but will never contact you directly or share your number with third parties.
How will your data be used?
The Restaurant may use your phone number to send you SMS messages about their specials, promotions, services, or updates. When you sign up, you are consenting to receive messages from that specific Restaurant.
How to opt-out or request deletion?
You can reply STOP to any SMS message from the Restaurant to unsubscribe. To request complete deletion of your data, contact the Restaurant directly or email us at privacy@rezform.com with the phone number and Restaurant's page URL, and we will process the deletion request.
Will Rezform contact you?
No. Rezform will never send marketing messages, promotions, or any other communications to phone numbers collected through capture pages. We only send a one-time verification code (OTP) during the signup process.
Information for Restaurants
If you have a Rezform account and create capture pages:
Your Account Data
We collect your email address, password (stored hashed), handle, and optional profile information. This data is used to provide and improve the service. We will never sell, distribute, or lease your personal information to third parties.
Your Responsibilities as Data Controller
When you collect phone numbers through your capture pages, you become the data controller for that subscriber data. This means you are responsible for:
- • Complying with applicable data protection laws (GDPR, CCPA, etc.)
- • Only using collected phone numbers for purposes the subscriber consented to
- • Providing a way for subscribers to opt-out (e.g., STOP to unsubscribe)
- • Responding to data access, correction, or deletion requests from subscribers
- • Keeping subscriber data secure when exported from Rezform
Subscriber Data You Collect
Phone numbers collected through your capture pages will remain private and confidential. We will never contact these phone numbers ourselves. We will never sell, rent, loan, or distribute subscriber phone numbers to third parties.
You can delete subscribers at any time, and their data will be permanently removed from our database. You can also export your subscriber list as a CSV file at any time.
OTP Verification
We use SMS One-Time Password (OTP) verification to ensure that phone numbers collected through capture pages are valid and owned by the person signing up. This verification SMS is the only message Rezform sends to subscriber phone numbers.
OTP verification is processed through our SMS provider (Twilio) and is subject to their privacy policy. OTP codes are temporary and are not stored after verification is complete.
Data Security
We take security seriously. All site communication occurs over an SSL encrypted channel, so your login credentials and data cannot be intercepted by third parties.
Up-to-date security patches
We regularly update our server software to minimise the risk of vulnerabilities.
Password hashing
We don't store passwords in plaintext, but use strong salted hashes to minimise the consequences of a data breach.
Encrypted data storage
Phone numbers are encrypted at rest in our database.
Cookies

Session Cookie
Used to uniquely identify a user, allow them to login to the site and maintain their session. Essential for site operation.
Authentication Token
Stores your authentication state securely to keep you logged in.
Communications from the Site
We may from time-to-time send emails to users of our site (but not to the phone numbers they may have collected). These may include system maintenance updates, account notifications, or service updates. We will never send advertisements on behalf of third parties.
Your Rights Under GDPR
If you are located in the European Economic Area (EEA) or the United Kingdom, you have certain data protection rights:

- • Right to Rectification: You can request that we correct inaccurate data.
- • Right to Erasure: You can request that we delete your personal data.

Data Retention

MV TEST SOLUTIONS LIMITED reserves the right to terminate, suspend, or restrict any account at any time, for any reason or no reason, with or without notice, and without liability. Upon termination:
- • All personal data associated with your account will be permanently deleted
- • All subscriber data you have collected will be permanently deleted
- • This data is unrecoverable and we accept no liability for any losses
We recommend regularly exporting your subscriber data to prevent loss in the event of account termination.
Limitation of Liability
To the maximum extent permitted by applicable law, MV TEST SOLUTIONS LIMITED, Rezform, and its directors, officers, employees, and agents shall not be liable for:
- • Any loss, damage, or unauthorized access to your data
- • Any data breaches or security incidents beyond our reasonable control
- • How Restaurants use subscriber data they collect through the Service
- • Third-party services, including SMS providers
- • Data loss resulting from account termination
While we implement reasonable security measures, we provide the Service "as is" without warranties of any kind. You use the Service at your own risk.
